Case Management Services for Adults

DDSD Services Coordinators assist adult clients with developmental disabilities in getting their needs met. They are responible for monitoring a client's services in residential sites and those clients who have in-home support plans. Specifically, Services Coordinators work to ensure that the Individual Support Plan is being followed, there are no health and safety issues, medical issues are being addressed and a person's money is being used appropriately. 

Case Management Services for Children and Young Adults

The DDSD Children and Young Adults Case Management Services Unit provides case management services for individuals who meet eligibility criteria for a developmental disability. All services and funding available are based on the criteria found in the Oregon Administrative Rules (OARs)

The children we serve live in a variety of settings. We have clients who live at home with families, children in foster care and children who live in residential homes such as proctor care or group homes. Each child we serve has an Individual Support Plan that outlines his/her support needs. DDSD Services Coordinators work closely with families to help children and young adults progress towards community integration, educational achievement, and transition into adulthood. This unit collaborates with public school systems, Oregon Children, Adults and Families Division (CAF), and other community agencies serving children.
