Case Management Services for Adults

DDSD Services Coordinators assist adult clients with developmental disabilities in getting their needs met. They are responsible for monitoring a client's services in residential sites and those clients who have in-home support plans. Specifically, Services Coordinators work to ensure that the Individual Support Plan is being followed, there are no health and safety issues, medical issues are being addressed and a person's money is being used appropriately.

Comprehensive Services for Adults

Comprehensive In-Home Support Services enable individuals aged 18 years or older with developmental disabilities to continue to live in their own homes or in their family homes. Services include assistance in determining support needs; finding and arranging resources and personal support services to meet the individual's needs; assisting individuals to make informed decisions about personal support needs and support providers; and monitoring and improving the quality of personal supports. 

Group Home Residential Facilities are services that provide care, training, and support services delivered in neighborhood homes to individuals with developmental disabilities who require 24-hour care, supervision, and training. 

Supported Living Services are individualized supports delivered in a personalized manner to people with developmental disabilities. Services include care, training, and support that promote opportunities for individuals to live in their own homes and apartments, and to be a part of and participate in the communities in which they live. 

Employment and Community Inclusion Services are out-of-home employment or community training services and related supports, delivered to individuals aged 18 or older with developmental disabilities, to improve the individual's productivity, independence and integration in the community. 

DD Foster Homes are residential care and services delivered to individuals with developmental disabilities in foster homes. Services include 24-hour supervision, the provision of room and board, and assistance with the activities of everyday living, from grooming and eating to participation in leisure activities and access to services which help the individual develop appropriate skills to increase or maintain their level of functioning. 

Services for Adults Not in Funded Services

Adults not in funded services include those in or receiving residential, Brokerage, vocational, and/or transportation services. DDSD Services Coordinators are responsible for ensuring the health and safety of these clients and referring them to available community resources to meet their needs. Areas of concern are adequate housing, medical and mental health supports, and assistance with the legal system. Services Coordinators support may include attending appointments or court hearings to ensure adequate representation of client needs and understanding of subsequent needed actions. 

Brokerages

Brokerages are contractors with the State of Oregon who provide services to adults age 18 an over who are living on their own. This can mean in their own home or with their parents. Brokerages handle most of the day-to-day issues of the people they serve and help them develop their support plans. Caseworkers at the Brokerages are called Personal Agents." Clients direct their own services and have the right to choose what is appropriate for themselves, based on their own goals. The Personal Agent serves as a facilitator. Individuals are encouraged to seek input from friends, parents, teachers, or other known and trusted people in choosing their supports and providers.
